Form to be completed by families of students who request a bus daily.
Welcome! We are excited about the upcoming school year and look forward to a safe and timely transportation service for your family.
PLEASE be sure to use your student's LEGAL NAME when completing the form.
Once the form is submitted, please allow up to 3-5 school days for processing. Then, visit the Sumner County Schools Transportation Page for Bus Stop Lookup to check your student's assigned bus information.

Please fill out this form if:
- Student did not ride the bus during the last 2 months of the 2024-25 school year
- New student requiring transportation
- Split household
- Alternate site (daily transportation to or from grandparents or a sitter)
- School-to-school transfers (Full-Time Staff only)
Important Notes:
- Students are required to be at the bus stop 5 minutes prior to the scheduled arrival time.
- Bus stop locations are subject to change every year. Please confirm the location of your student's bus stop, bus number, and stop time using the link above.
- PLEASE SUBMIT ONE FORM PER STUDENT NEEDING TRANSPORTATION.
